Check with your church for available ‘Life Choices Clinic Baby Bottles’ or get one directly from us by visiting our clinic. - Fill a Baby Bottle
Fill out the small slip provided inside the bottle(s). This allows us to keep track of your giving and receipts. Fill the bottle with spare change, checks, or cash. - Return your Baby Bottle
Bring bottle(s) back to a participating church on collection day (Father’s Day) or to Life Choices Clinic during business hours. (open M-Th). - Change lives!
